Q: Should this include the Nominee’s military accomplishments as well as civilian accomplishments?
A: Employment and education history should include both military and civilian activity.

Q: My nominee received both his undergraduate and masters degree while on active duty, does this count?
A: Yes, employment and education history should include military activity.

The Nomination Package for Submission

Q: Would you recommend the nomination package be placed in a binder?
A: No.

Q: Is there another way to submit a nomination other than by this Form?
A: No, not at this time.

Q: Once I’ve filled out the form what do I do with it?
A: You either:
(1) SAVE it to your hard drive and send it as an email attachment to: FVHOF@FDVA.STATE.FL.US;
