The Nomination Package for Submission

1. Would you recommend the nomination package be placed in a binder?

No.

2. Is there another way to submit a nomination other than by this Form?

No, not at this time.

3. Once I’ve filled out the form what do I do with it?

You either: (1) SAVE it to your hard drive and send it as an email attachment to: FVHOF@FDVA.STATE.FL.US (2) PRINT it out and mail it to: Florida Department of Veterans’ Affairs; Attention: Veterans’ Hall of Fame; The Capitol, Suite 2105; 400 South Monroe Street; Tallahassee, Florida 32399.

Nominee’s Education and Training Accomplishments

1. Should this include the Nominee’s military accomplishments as well as civilian accomplishments?

Employment and education history should include both military and civilian activity.

2. My nominee received both his undergraduate and masters degree while on active duty, does this count?

Yes, employment and education history should include military activity.

1. How do I locate military records from the government about someone I’d like to nominate?

Contact the National Archives Veterans’ Service Records.

2. Where do I get a person’s DD-214?
